Program Description:
This means you are requesting to release a 3rd Party to receive information (i.e. application, academic, etc.) or documents (Alert: I-20s are not allowed to be given to anyone except the student directly) on your behalf. A 3rd Party could be a relative, friend, agent, etc. that you are giving permission to Citrus College to give personal information and documents to on your behalf.
If this is what you are requesting, complete the RELEASE OF INFORMATION FORM and upload your signed copy, then click the "Submit" button.
